J-Card Request Form

This form is intended primarily for Advanced Academic Programs' students enrolled in Washington, D.C.-based courses. Once you have been notified your J-Card is available, please collect it at 555 Pennsylvania Avenue, NW, Washington, D.C. 20001.

You can find your Hopkins ID in SIS or by going to my.jh.edu and signing in using your JHED credentials. Once you've signed in, click the myProfile button on the left side bar and your Hopkins ID should be the first thing listed.
Please provide the email address issued by Johns Hopkins University.
